What you will do:
- Assess the needs of individuals and families affected by domestic abuse and identify appropriate support pathways.
- Develop and implement personalised support plans in partnership with service users and relevant agencies.
- Work collaboratively with community partners and multidisciplinary teams to improve outcomes.
- Advocate for clients’ rights and coordinate multi-agency meetings to ensure effective support.
- Provide emotional support, practical guidance, and safety planning to empower clients.
- Maintain accurate and up-to-date case records in line with statutory requirements and organisational policies.
- Participate in team meetings and contribute to the continuous improvement of services.
