What you will do:
- Work as an integral member of the Integrated Community Nursing Team providing comprehensive nursing care for patients in their own community settings.
- Promote a positive image of community nursing and deliver an exceptional service to all patients.
- Maintain patient confidentiality and adhere to all necessary data protection regulations while accurately managing both written and electronic records.
- Communicate effectively with patients' families and carers, providing necessary support and guidance in collaboration with the District Nurse Team Leader.
- Instigate, participate in, and evaluate care plans as part of an Integrated Locality Service, maintaining high standards of patient records.
- Ensure the provision and safe use of required nursing equipment, instructing patients and their families on proper use.
- Engage in informal teaching for patients and carers as well as assisting in the induction and training of new staff.
- Seek opportunities to rotate through the Emergency Department and Medical Assessment Unit, as guided by the District Nurse Team Leader.
