What you will do
- Manage a caseload of approximately 55 clients by providing comprehensive support through key work sessions, care planning, and risk assessment.
- Keep meticulous records of client interactions and progress using SystmOne, ensuring smooth communication with partner agencies.
- Conduct outreach work in Shirebrook confidently and independently, promoting recovery and positive change in the community.
- Collaborate closely with colleagues and external agencies to provide holistic, integrated care for clients.
- Drive to various work sites with your vehicle, demonstrating flexibility and adaptability to meet the demands of the role.
