What you will do

  • Perform specialist assessments, rehabilitation, and discharge planning for complex patients.
  • Conduct clinical triage of referrals into Intermediate Care services.
  • Complete trusted assessor assessments.
  • Assess, order, and review specialist equipment.
  • Coordinate complex discharge pathways from acute and community settings.
  • Manage a specialist caseload and coordinate patient flow.
  • Lead case conferences to support safe and timely discharge.
  • Provide specialist clinical advice to patients, carers, and professionals.
  • Support admission avoidance and facilitate earlier discharge.
  • Make recommendations regarding healthcare packages and onward referrals.
  • Liaise with multidisciplinary teams, community services, and care providers.
  • Maintain accurate clinical records and documentation.
  • Support service development, audits, and quality improvement initiatives.
  • Mentor, supervise, and appraise junior staff members.
  • Deliver training on discharge planning and Intermediate Care pathways.

Requirements

  • HCPC Registration: Must be a qualified Physiotherapist or Occupational Therapist with current HCPC registration.
  • Relevant Experience: Previous experience in discharge planning and community-based settings is essential.
