What you will do
- Complete specialist assessment, rehabilitation, and discharge planning for complex patients.
- Undertake clinical triage of referrals into Intermediate Care services.
- Complete trusted assessor assessments to support patient care.
- Assess, order, and review specialist equipment for patient needs.
- Coordinate complex discharge pathways from acute and community settings.
- Manage a specialist caseload and coordinate patient flow efficiently.
- Lead case conferences to support safe and timely discharge.
- Provide specialist clinical advice to patients, carers, and professionals.
- Support admission avoidance and facilitate earlier discharge.
- Make recommendations regarding healthcare packages and onward referrals.
- Liaise with multidisciplinary teams, community services, and care providers.
- Maintain accurate clinical records and documentation.
- Support service development, audits, and quality improvement initiatives.
- Mentor, supervise, and appraise junior staff members.
- Deliver training on discharge planning and Intermediate Care pathways.
