What you will do

  • Reablement Support: Provide specialist occupational therapy support across a 16-bed reablement service.
  • Discharge Planning: Facilitate safe and effective discharges from hospital into community settings.
  • Assessment: Assess individuals' functional abilities and identify support needs within community environments.
  • Care Planning: Develop personalised reablement plans aimed at maximising independence and daily living skills.
  • Equipment and Adaptations: Recommend and arrange appropriate equipment and environmental adaptations where required.
  • Admission Prevention: Support community step-up admissions and help prevent avoidable hospital admissions.
  • Multi-Disciplinary Working: Collaborate closely with healthcare professionals to deliver coordinated care and positive outcomes.
  • Professional Practice: Work in accordance with HCPC standards, maintaining high levels of professional and ethical practice.

Qualifications / Requirements

  • HCPC Registration: Must be a registered Occupational Therapist.
  • Experience: Previous Occupational Therapy experience within community, rehabilitation, or reablement services is
