What you will do:
Conduct community assessments in a local authority setting, identifying needs and recommending both standard and specialist equipment.
Plan and facilitate equipment and minor adaptations to enhance safety and functionality within clients' homes.
Collaboratively work with clients, families, Social Workers, and Assessment and Review Officers to create individualised support plans.
Maintain thorough and accurate records in line with professional and legal standards.
