What you will do:
- Undertake statutory assessments, reviews, and support planning for young people transitioning into Adult Social Care services.
- Complete Care Act Assessments and contribute to Education, Health and Care Plan (EHCP) reviews where appropriate.
- Develop person-centred support plans that promote independence, wellbeing and positive life outcomes.
- Work collaboratively with Children's Services, Adult Social Care teams, health professionals, education providers, and voluntary sector organisations.
- Manage a complex caseload, ensuring interventions are delivered within statutory timescales and professional standards.
Person Specification:
Qualifications
- Degree in Social Work, CQSW, DipSW or an equivalent recognised Social Work qualification.
- Evidence of continuing professional development relevant to adult and transition services.
Registration & Compliance
- Registration with Social Work England.
Experience
- Post-qualification experience within Children's Services, Adult Social Care, or Transition Services.
- Experience supporting young people with disabilities, autism, learning disabilities, or complex needs.
- Experience undertaking Care Act Assessments and managing statutory social work caseloads.
