What you will do

  • Conduct therapy assessments in patients' own homes following their discharge from the hospital, ensuring a safe transition and influencing ongoing care pathway decisions.
  • Work collaboratively within a multidisciplinary team to provide high-quality care and support.
  • Liaise with patients, families, and carers to develop and implement care plans tailored to individual needs.
  • Document assessments and interventions accurately, ensuring compliance with professional and legal standards.
  • Support service development initiatives within the Discharge to Assess framework.

Requirements

In addition to being an HCPC registered Occupational Therapist, you will need to be a competent car driver as this role involves travel between various locations and patient homes.
