You will play a vital role in supporting patient recovery following discharge and promoting long-term wellbeing in a community setting. Key Responsibilities Conduct comprehensive assessments and deliver targeted interventions for patients post-discharge. Manage triage of referrals from both discharge hubs and the Home First initiative. Handle telephone referrals , ensuring accurate communication and appropriate follow-up actions. Collaborate effectively with a multidisciplinary team to ensure high standards of patient care. Develop and implement individualised care plans tailored to patients\’ specific rehabilitation needs. Requirements HCPC registration as a qualified Physiotherapist is essential. Must be a car driver , with the ability to travel between community locations as required.
