What you will do:
- Manage a caseload of adults with severe and enduring mental health conditions, delivering comprehensive assessment, intervention, treatment, and recovery support.
- Undertake mental health, risk, and safeguarding assessments to develop person-centred care plans that meet the unique needs of each client.
- Act as Care Coordinator under the Care Programme Approach (CPA), organising effective planning, review, and care evaluation.
- Provide evidence-based nursing interventions in community settings and monitor treatment outcomes to ensure optimal results.
- Administer, monitor, and evaluate medication while identifying and addressing any physical or mental health concerns.
Person Specification:
Qualifications:
- Registered Mental Health Nurse (RMN)
- Degree or Diploma in Mental Health Nursing
- Continuous professional development in community mental health practice
Registration & Compliance:
- Current NMC registration with no restrictions
Experience:
- Significant post-registration experience in community mental health services, CMHTs, or similar secondary care settings
- Expertise in working with adults presenting complex mental health needs, including psychosis, severe mood disorders, and dual diagnosis
