What you will do:
Coach, develop, and manage a group of social workers and support staff, ensuring the best outcomes for adult residents in need of care and support.
Oversee complex casework and assess the care and support needs of clients to promote independence and wellbeing.
Implement Making Safeguarding Personal (MSP) practices and oversee performance management within the team.
Quality assure the work of social workers and care assistants, ensuring excellence in service delivery.
Engage in multidisciplinary meetings as part of the Care Closer to Home initiative with the Integrated Care Board (ICB).
Foster partnerships with local care homes, health partners, and statutory bodies to create integrated care pathways.
