What you will do
- Undertake Care Act 2014 assessments, reviews and support planning for adults requiring social care intervention following hospital admission.
- Facilitate safe, timely and effective hospital discharges through collaborative working with health professionals, families, carers and community services.
- Complete Mental Capacity Act assessments, Best Interest decisions and risk assessments where required.
- Manage a caseload of adults with complex care and support needs, ensuring outcomes are person-centred and strengths-based.
- Undertake safeguarding enquiries and contribute to risk management processes in accordance with relevant legislation and local procedures.
Person Specification
- Degree in Social Work, CQSW, DipSW or an equivalent recognised Social Work qualification.
- Evidence of continuing professional development relevant to Adult Social Care.
- Current registration with Social Work England SWE.
- Post-qualification experience within Adult Social Care.
- Experience of working within a hospital discharge, integrated care, community social work or reablement service.
