36 hours per week . This role involves working two days in the office at 38-46 Station Road, Wood Green, London, with the remainder of the time working remotely.
We are seeking an experienced social worker with a background in Leaving Care who has the expertise to complete various assessments and plans to a high standard.

- Conduct and complete Pathway Plans, Child and Family Assessments, and update Case Summaries with precision.
- Prepare and maintain detailed and accurate records, including Completing Chronology and PEP.
- Collaborate closely with young adults and their families to meet their unique needs and support their transition to independence.
- Act as a key liaison between agencies, advocating effectively for young adults within the social care system.
- Participate in an initial two-week office-based induction and mandatory training to ensure you are well-prepared and supported in your role.
