What you will do
- Conduct assessments and develop care plans under the Care Act and Mental Capacity Act, ensuring individuals receive appropriate support.
- Collaborate with a multidisciplinary team, including health services, to facilitate the safe discharge of adults from acute hospital settings.
- Engage with adults, families, and carers to navigate complex safeguarding situations and ensure effective risk management.
- Lead in managing complex cases, ensuring timely transitions with services in place for patient support at home.
- Provide additional MCA work when needed and liaise daily with relevant parties to ensure comprehensive care and support.
