What you will do

  • Undertake Care Act assessments, reviews and reassessments for adults with varying health and social care needs.
  • Develop and review person-centred care and support plans, ensuring outcomes are clearly identified and monitored.
  • Work collaboratively with health professionals, including GPs, nurses, and mental health professionals.
  • Serve as a liaison between health and social care services, ensuring effective communication and information sharing.
  • Support adults in maximising independence, choice, control, and quality of life.
  • Identify safeguarding concerns and take timely, appropriate actions in accordance with procedures.
  • Coordinate hospital discharge and transitions between services when required.
  • Engage with service users, families, and carers to identify suitable community resources.
  • Consider eligibility for services and recommend appropriate care options.
  • Maintain detailed, timely case records and documentation.
  • Prepare professional reports and contribute to multidisciplinary meetings and reviews.
  • Assist in preventing unnecessary hospital admissions and promote safe community reintegration.
  • Advocate for strengths-based and preventative approaches to adult social care.
  • Support individuals with complex or multiple needs, ensuring equality and dignity are upheld.
  • Manage a caseload efficiently, prioritising according to risk and statutory requirements.
