What you will do
- Manage a caseload of young people transitioning into adulthood, providing essential guidance and support
- Undertake assessments, reviews, and care planning in accordance with legislative and organisational requirements
- Collaborate with individuals, families, carers, and multi-agency professionals to develop holistic support plans
- Champion independence, choice, and control through strengths-based practice
- Safeguard vulnerable adults, ensuring risks are appropriately assessed and managed
- Maintain accurate and timely case recordings and documentation
- Positively contribute to team development and service improvement initiatives
To shine in this role, you must:
- Hold a recognised Social Work qualification and be registered with Social Work England
- Have a minimum of two years’ post-qualification experience in social work practice
- Demonstrate experience of using LAS (Liquidlogic Adult System)
- Possess a sound understanding of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005
- Boast strong assessment, communication, and case management skills
- Hold a valid driving licence and have access to a vehicle for work purposes
- Reside within one hour’s travelling distance of Rotherham
