What you will do:

• Manage your own caseload of adults presenting with a range of medical conditions, disabilities, and complex needs.
• Undertake holistic Occupational Therapy assessments and interventions designed to promote independence, wellbeing, and safety.
• Conduct comprehensive functional and Occupational Therapy assessments within community settings.
• Assess and recommend appropriate equipment, assistive technology, and home adaptations.
• Undertake manual handling and risk assessments and recommend suitable equipment or interventions where required.
• Support individuals to maximise their independence through reablement, prevention, and strengths-based approaches.
• Work collaboratively with health professionals, housing teams, care providers, and other multidisciplinary services.
• Complete Care Act assessments and develop appropriate support plans where required.
• Maintain accurate, timely, and comprehensive case records and professional documentation.
• Contribute to safeguarding, risk management, and positive outcomes for adults living within the community.
• Use professional judgement to identify appropriate interventions and coordinate effective care and support.
• Recognised qualification in Occupational Therapy.
• Current HCPC registration.
• Previous experience undertaking Occupational Therapy assessments and interventions.
• Experience managing an independent caseload.
• Strong understanding of Adult Social Care practice and principles.
• Ability to assess complex needs and formulate appropriate interventions.
• Strong communication, assessment, organisational, and report-writing skills.
• Ability to work effectively both autonomously and as part of a multidisciplinary team.
• Commitment to person-centred, strengths-based, and outcome-focused practice.
