What you will do:
- Conduct Care Act assessments to determine individuals’ needs and appropriate care and support.
- Manage safeguarding cases and work on risk management plans for those with complex mental health needs.
- Collaborate with multi-disciplinary teams, including health partners, to deliver holistic care and support.
- Make autonomous, informed decisions on complex cases to improve service user outcomes.
- Maintain comprehensive records and ensure all documentation meets statutory requirements.
