What you will do
- Undertake proportionate, strengths-based assessments of adults’ care and support needs.
- Develop person-centred care and support plans that promote independence and wellbeing.
- Respond to changing circumstances through timely reviews and reassessments of care arrangements.
- Conduct adult safeguarding enquiries in accordance with relevant legislation and procedures.
- Apply the principles of the Mental Capacity Act 2005 within assessments and care planning.
- Manage an allocated caseload and maintain accurate, comprehensive case records.
- Participate in multidisciplinary meetings and work collaboratively with partner agencies and professionals.
- Support adults to maximise choice, control, independence, and positive outcomes.
- Work with families, carers, advocates, and community organisations to coordinate effective support.
