What you will do
- Conduct strengths-based assessments of adults’ needs.
- Identify eligible needs and desired outcomes in line with the Care Act 2014.
- Develop person-centred plans that promote choice, control, independence, and wellbeing.
- Implement, review, and amend care and support plans based on assessed needs.
- Manage a varied caseload effectively, prioritising work according to risk and urgency.
- Maintain accurate and comprehensive case records in accordance with statutory and organisational requirements.
- Respond appropriately to safeguarding concerns and undertake safeguarding duties where required.
- Work collaboratively with partner agencies to develop and implement safeguarding plans.
- Apply the principles of the Mental Capacity Act in assessments, planning, and decision-making.
