What you will do:
- Conduct comprehensive assessments of adults referred to the Adults MASH Team to identify care, support and safeguarding needs.
- Liaise with partner agencies to gather and share information, ensuring a coordinated multi-agency approach to adult safeguarding.
- Manage complex cases, undertaking safeguarding enquiries, assessing risks and developing appropriate intervention and support plans.
- Provide professional guidance and support to colleagues and partner organisations, contributing to effective risk management and positive outcomes for adults.
- Maintain accurate case records, ensuring all assessments, actions and professional decisions are clearly documented in line with the Care Act and statutory requirements.
