What you will do:
- Manage complex care programmes for adults with learning disabilities, autism, and older adult needs.
- Coordinate care across health and social care services, ensuring a seamless delivery of support.
- Assess complex cases and develop personalised, person-centred care plans.
- Work collaboratively with a multidisciplinary team to ensure comprehensive care for service users.
- Support service users, carers, and families, providing guidance and assistance as needed.
- Coordinate interventions and monitor outcomes, making adjustments to care plans as needed.
- Maintain accurate records and documentation, ensuring compliance with relevant policies and procedures.
- Contribute to risk management and care planning processes, enhancing the safety and effectiveness of care.
Requirements:
- Degree in Occupational Therapy and HCPC Registration.
- Experience with learning disabilities, autism, and older adult care.
- Demonstrated ability in complex case management and care coordination.
- Strong skills in multidisciplinary teamwork, communication, and documentation.
- Experience in NHS Mental Health, community settings, or a case management qualification.
