What you will do
- Deliver the Individual Placement and Support model to help individuals with mental health needs gain and stay in employment.
- Work closely with health and social care teams in Merton and Sutton to devise tailored employment plans for service users.
- Engage with local employers to unlock opportunities, acting as a bridge for service users aiming to step into suitable roles.
- Monitor and review user progress regularly, adapting plans to ensure sustainable employment outcomes.
- Provide ongoing support to both employers and service users, addressing any issues that arise and ensuring a smooth and supportive transition into the workplace.
