What you will do

  • Deliver high-quality clinical pharmacy services across hospital wards.
  • Conduct comprehensive medicines reconciliation and medication history assessments.
  • Perform clinical screening of prescriptions and drug charts to ensure patient safety.
  • Complete TTA screening and detailed discharge medication reviews.
  • Provide specialist pharmaceutical advice to multidisciplinary healthcare teams.
  • Support safe and effective medicines optimisation across the service.
  • Review and develop medication therapy plans to improve patient outcomes.
  • Participate in ward rounds and contribute to clinical decision‑making processes.
  • Ensure adherence to medicines management policies, procedures, and best practice guidance.
  • Maintain accurate clinical documentation and pharmacy records.

Requirements

  • GPhC Registration: Qualified Pharmacist with current General Pharmaceutical Council registration.
  • NHS Experience: Proven NHS experience, particularly within hospital ward-based pharmacy services.
  • Medicines Reconciliation: Experience completing drug histories and medicines reconciliation.
  • Clinical Screening: Strong experience with clinical screening and TTA reviews.
  • EPMA Knowledge: Familiarity with EPMA systems, preferably Cerner EPMA.
  • Clinical Expertise: Excellent clinical pharmacy knowledge and decision-making skills.
  • Communication Skills: Strong interpersonal, organisational, and multidisciplinary working skills.
  • Band 7 or Band 8a Experience: Previous experience working at Band 7 or Band 8a level.
  • Acute Hospital Experience: Experience within an acute hospital pharmacy environment.
