We are looking for a dynamic Health and Wellbeing Practitioner to join our team at HMP Brixton. As part of our organisation, you will be working in a supportive and forward-thinking environment that is passionate about making a real difference in individuals’ lives. You’ll be working full-time, 35 hours per week, on-site in a dynamic and challenging environment where your work will have a direct and positive impact on the wellbeing and recovery journeys of those in custody.

What you will do

  • Manage a caseload of service users with substance misuse needs.
  • Conduct risk assessments and develop tailored care plans.
  • Deliver one-to-one and group interventions to support recovery.
  • Provide harm reduction advice and guidance.
  • Collaborate with partners across healthcare, housing, and employment to support seamless through-care and resettlement.
  • Maintain detailed and accurate records in line with organisational standards.
  • Support the service in achieving targets, key performance indicators, and continuous improvement.

Requirements

  • Experience supporting individuals with substance misuse issues.
  • Confidence in delivering tailored one-to-one and group interventions.
  • Ability to build effective partnerships with healthcare providers and external agencies.
  • A resilient, compassionate, and motivated approach to supporting recovery.
  • Experience working in a prison environment is desirable.
