What you will do:
- Provide expert intervention to enhance patient recovery and assist in hospital discharge processes.
- Apply your knowledge in intermediate care, discharge to assess and assessment frameworks to support patient transitions.
- Utilise SystmOne to maintain accurate and up-to-date patient records.
- Travel to various sites and patients’ homes as required, ensuring high-quality care continuity.
Additional requirements:
- Registered with HCPC.
- Must possess a full driving license and access to a vehicle for travel.
