Responsibilities
- Deliver evidence-based dietetic services tailored to residents’ nutritional needs and medical conditions.
- Manage a varied caseload, including cases related to frailty, diabetes, oncology, and more.
- Perform comprehensive dietetic assessments and implement personalised nutritional care plans.
- Communicate effectively with residents, families, GPs, and care teams, promoting a collaborative care approach.
- Respond promptly to urgent referrals and support care continuity outside of regular hours when necessary.
- Provide clinical input into the prescription of oral and enteral nutrition, in line with best practices.
- Coordinate care plans for individuals transitioning between hospital and home.
- Partner with the Chef Team to enhance menu offerings, aligning with nutritional standards and residents’ needs.
