Responsibilities

  • Deliver evidence-based dietetic services tailored to residents’ nutritional needs and medical conditions.
  • Manage a varied caseload, including cases related to frailty, diabetes, oncology, and more.
  • Perform comprehensive dietetic assessments and implement personalised nutritional care plans.
  • Communicate effectively with residents, families, GPs, and care teams, promoting a collaborative care approach.
  • Respond promptly to urgent referrals and support care continuity outside of regular hours when necessary.
  • Provide clinical input into the prescription of oral and enteral nutrition, in line with best practices.
  • Coordinate care plans for individuals transitioning between hospital and home.
  • Partner with the Chef Team to enhance menu offerings, aligning with nutritional standards and residents’ needs.
