What you will do:

  • Undertake Care Act 2014 assessments, reviews and support planning for adults requiring social care intervention following hospital admission.
  • Facilitate safe, timely and effective hospital discharges through collaborative working with health professionals, families, carers and community services.
  • Complete Mental Capacity Act assessments, Best Interest decisions and risk assessments where required.
  • Manage a caseload of adults with complex care and support needs, ensuring outcomes are person-centred and strengths-based.
  • Undertake safeguarding enquiries and contribute to risk management processes in accordance with relevant legislation and local procedures.

Person Specification:

  • Degree in Social Work, CQSW, DipSW or an equivalent recognised Social Work qualification.
  • Evidence of continuing professional development relevant to Adult Social Care.
  • Current registration with Social Work England SWE.
  • Post-qualification experience within Adult Social Care.
  • Experience of working within a hospital discharge, integrated care, community social work or reablement service.
