What you will do
- Engage in Care Act assessments and reviews.
- Assess individuals after hospital admission to identify ongoing care and reablement needs.
- Support safe and timely hospital discharges.
- Develop person-centred and solution-focused support plans.
- Collaborate with health professionals, families, carers, and agencies to coordinate care.
- Identify and arrange appropriate care and support, liaising with Brokerage as needed.
- Manage complex cases, assessing and managing risk levels effectively.
- Utilise professional social work judgement in complex situations.
- Encourage independence by helping people to achieve personal goals.
- Maintain high standards in social work practice and ensure accurate case recording.
Requirements
- Qualified Social Worker registered with Social Work England.
- Strong background in Adult Social Care.
- Desirable experience in hospital discharge or community social work.
- In-depth knowledge of the Care Act and adult social care legislation.
- Proficiency in managing complex cases and risk.
- Ability to work collaboratively within a multi-disciplinary environment.
- Strong communication and assessment capabilities.
