What You Will Do:
- Manage a caseload efficiently whilst providing support and guidance to adults living at home.
- Conduct Care Act Assessments, assemble Support Plans, and engage in Mental Capacity Assessments and Best Interest Decisions.
- Signpost individuals to internal and external services as necessary to promote wellbeing and independence.
- Forge effective professional relationships with multidisciplinary teams and local health and social care services for a coordinated support strategy.
- Communicate expertly, ensuring information is clear and correct to better support individuals and families.
