What you will do:
- Deliver timely, high-quality Care Act assessments, carer assessments, and support planning to help individuals maintain independence.
- Manage a diverse caseload including high-risk adult safeguarding and complex transitions from children’s services.
- Collaborate with multi-agency partners like the Integrated Care Board and Continuing Healthcare to ensure seamless care.
- Apply mental capacity legislation effectively, managing Deprivation of Liberty Safeguards frameworks and conducting capacity assessments.
- Proactively handle risk through multi-agency safety planning, crisis intervention, and preventative escalation processes.
- Participate in panels and funding assessments to secure joint care funding.
- Operate independently managing complex community cases while receiving necessary oversight for high-risk issues.
- Conduct face-to-face meetings and develop comprehensive, evidence-based reports.
- Engage with service users and families to negotiate care packages and navigate difficult transitions.
- Maintain impeccable case records on the council’s system as per professional standards.
- Be part of team duty rotas to manage urgent queries and crisis interventions.
- Provide mentorship and support to less experienced colleagues and students.
- Contribute to service improvement initiatives and team development.
