What you will do
- Conduct comprehensive social care assessments to identify eligible needs and determine appropriate support under the Care Act 2014.
- Develop, implement, and review person-centred care and support plans that promote independence and wellbeing.
- Work collaboratively with service users, families, carers, and partner organisations to achieve positive outcomes.
- Identify carers and undertake Joint Carers Assessments to ensure appropriate support networks are in place.
- Act as an Enquiry Practitioner for safeguarding concerns, ensuring vulnerable adults are protected from abuse and neglect.
- Undertake Mental Capacity Assessments and contribute to Best Interest decision-making in line with the Mental Capacity Act 2005.
- Manage a caseload effectively, ensuring timely interventions, reviews, and assessments.
- Work closely with health professionals, care providers, and community organisations to provide coordinated support.
