What you will do

  • Collaborate within a multi-agency framework to provide holistic care planning.
  • Safeguard the welfare of vulnerable adults, ensuring their safety and well‑being.
  • Manage a diverse and complex caseload, handling all care coordination duties.
  • Conduct Mental Capacity Assessments and enforce the Care Act 2014 requirements.
  • Foster independence through relationship-based and person-centred approaches.
  • Inform senior practitioners of significant events and coordinate service operations efficiently.
  • Offer expert advice to colleagues and stakeholders within and outside the NHS.
  • Maintain comprehensive records as per Council Policy and Data Protection Laws.
  • Attend disciplinary meetings and achieve service goals through effective communication.
  • Ensure compliance with the Customer Service Excellence and Health and Safety standards.
  • Use social work methods to address personal and social challenges faced by clients.
