What you will do:
- Manage a caseload of individuals with complex mental health needs, supporting timely hospital discharge while addressing barriers to discharge.
- Engage in Section 117 aftercare planning and reviews, ensuring appropriate aftercare services are arranged.
- Arrange and coordinate care packages, placements, supported accommodation, and community support services to facilitate safe discharge from hospitals.
- Fulfil duties relating to local authority statutory requirements under The Care Act (2014), The Mental Health Act (83/07), and The Mental Capacity Act (2005).
- Report directly to your Senior Social Worker and work collaboratively within the team under the guidance of the Redwoods Mental Health Social Work Team Manager and AMHP Lead.
- Build strong relationships with customers, carers, and multidisciplinary teams across both statutory and voluntary services.
