This exciting locum opportunity offers full-time hours of 37 per week on a 12-week contract starting on 24th August 2026.

Working with adults aged 67 and over who have physical disabilities, sensory impairments, mental health needs, dementia, and age-related support requirements, you will play a pivotal role in promoting independence, safeguarding vulnerable adults, and delivering person-centred social care interventions.

What you will do:

  • Conduct comprehensive social care assessments to identify eligible needs and determine appropriate support under the Care Act 2014.
  • Develop, implement, and review person-centred care and support plans that promote independence and wellbeing.
  • Work collaboratively with service users, families, carers, and partner organisations to achieve positive outcomes.
  • Identify carers and undertake Joint Carers Assessments to ensure appropriate support networks are in place.
  • Act as an Enquiry Practitioner for safeguarding concerns, ensuring vulnerable adults are protected from abuse and neglect.
  • Undertake Mental Capacity Assessments and contribute to Best Interest decision-making in line with the Mental Capacity Act 2005.
  • Manage a caseload effectively, ensuring timely interventions, reviews, and assessments.
  • Work closely with health professionals, care providers, and community organisations to provide coordinated support.
  • Maintain accurate, timely, and legally compliant case records and documentation.
  • Promote strengths-based practice and support individuals to maximise independence wherever possible.
