What you will do
- Undertake strengths-based assessments and develop person-centred care and support plans.
- Manage a complex caseload while ensuring compliance with statutory duties and legislative requirements.
- Complete Mental Capacity Act assessments and Best Interest decisions where appropriate.
- Conduct reviews, reassessments, and safeguarding enquiries in accordance with local authority procedures.
- Work collaboratively with service users, carers, families, health professionals, and partner agencies.
- Prepare high-quality reports, assessments, and documentation, including court reports where required.
- Support adults to maximise independence, wellbeing, and community participation.
- Undertake risk assessments and implement effective risk management strategies.
- Contribute to service improvement initiatives and the ongoing development of Adult Social Care services.
