What you will do

  • Undertake comprehensive mental health assessments for individuals in acute crisis.
  • Complete urgent crisis and risk assessments in community settings, including patients' homes, emergency departments, police custody suites, and inpatient wards.
  • Formulate and implement person-centred crisis care plans using evidence-based interventions.
  • Deliver intensive home treatment as an alternative to psychiatric hospital admission.
  • Monitor mental state, medication effectiveness, physical health, and overall progress during episodes of crisis.
  • Administer and monitor prescribed medication, including depot injections.
  • Conduct Mental Health Act and Mental Capacity Act assessments where appropriate.
  • Provide rapid assessment and intervention for adults presenting in mental health crisis.
  • Act as a gatekeeper for acute psychiatric admissions.
  • Develop, implement, and review recovery-focused care plans.
  • Coordinate care with Community Mental Health Teams, GPs, Social Services, and more.
  • Lead on dynamic risk assessments and elevate safeguarding concerns.
  • Deliver brief psychological and recovery-focused interventions.
  • Provide clinical leadership and act as shift coordinator when required.
