What you will do:
- β Conduct outcome-focused assessments for adults with physical disabilities and age-related conditions.
- β Perform complex moving and handling assessments within the Front Door Team.
- β Evaluate physical dysfunction and functional ability to recommend appropriate complex equipment solutions.
- β Provide expert manual handling advice and intervention planning.
- β Support safe and effective care planning, managing cases independently.
- β Collaborate with multidisciplinary teams and partner agencies to ensure holistic care.
- β Maintain meticulous records and assessment documentation to support care delivery.
Requirements:
- β Degree or Diploma in Occupational Therapy, with HCPC registration.
- β Proven experience as a Senior Occupational Therapist in an Adult Social Care setting.
- β Extensive experience with complex moving and handling and equipment assessment.
- β Autonomous work capability with strong assessment and clinical reasoning skills.
- β Excellent communication and report-writing skills.
Desirable Requirements:
- β Previous experience within a Front Door Team or community Occupational Therapy setting.
- β Familiarity with Local Authority services and managing complex care assessments.
