What you will do:
- Assess and treat predominantly older adult patients while delivering community-based rehabilitation programmes
- Support patients to maximise independence and functional outcomes, ensuring high-quality care
- Complete comprehensive physiotherapy assessments and develop as well as review person-centred treatment plans
- Work effectively within multidisciplinary community teams to facilitate safe discharge and ongoing rehabilitation
- Maintain accurate clinical records and documentation
- Provide specialist advice to patients, carers, and healthcare professionals
- Manage an independent community caseload ensuring patient-centred care and effective treatment strategies
- Community rehabilitation setting focused on older adults
- Access to your own vehicle is necessary for community work, providing the freedom to manage your caseload across different locations
Requirements:
- Degree or Diploma in Physiotherapy
- HCPC registration
- Band 6 Physiotherapy experience with particular experience working with older adults
- Previous community rehabilitation experience with strong clinical assessment and reasoning skills
- Full UK driving licence with access to a vehicle for community visits
- Experience with NHS Scotland
- Community physiotherapy experience within multidisciplinary rehabilitation teams is preferred
