What you will do:
- Hold asset-based, person-centred conversations to identify strengths, risks, and needs
- Co-produce personalised support plans
- Formulate and execute plans to manage risk ensuring the achievement of desired outcomes
- Identify and intervene in alignment with Safeguarding Adult procedures when dealing with abuse allegations
- Respond effectively to emergencies, such as the breakdown of an existing care situation
- Conduct statutory assessments under the Care Act 2014, Mental Capacity Act 2005, and other pertinent legislation
Requirements of the Social Worker:
- Hold a degree or equivalent qualification in social work
- Current registration with Social Work England
- Substantial frontline experience within UK Social Services as a qualified Social Worker
