What you will do
- Conduct comprehensive assessments of service users' needs, strengths, risks, and goals.
- Develop, regularly review, and adapt individualised recovery and support plans.
- Deliver structured one-to-one interventions focusing on substance misuse and recovery.
- Assist clients in reducing or ceasing substance use and maintaining positive changes.
- Manage your assigned caseload efficiently with regular contact and accurate case recording.
- Identify and address safeguarding, risk, and health concerns effectively.
- Offer harm-reduction advice and promote safer choices for service users.
- Facilitate access to healthcare, housing, benefits, education, employment, and other essential services.
- Collaborate with external agencies and professionals to coordinate comprehensive care.
