What you will do

  • Conduct comprehensive assessments of service users' needs, strengths, risks, and goals.
  • Develop, regularly review, and adapt individualised recovery and support plans.
  • Deliver structured one-to-one interventions focusing on substance misuse and recovery.
  • Assist clients in reducing or ceasing substance use and maintaining positive changes.
  • Manage your assigned caseload efficiently with regular contact and accurate case recording.
  • Identify and address safeguarding, risk, and health concerns effectively.
  • Offer harm-reduction advice and promote safer choices for service users.
  • Facilitate access to healthcare, housing, benefits, education, employment, and other essential services.
  • Collaborate with external agencies and professionals to coordinate comprehensive care.
  • Encourage engagement with mutual aid, peer support, and community recovery opportunities.
  • Support relapse prevention and assist in developing sustainable recovery plans.
  • Maintain precise, confidential, and timely records in line with CGL standards.
  • Participate actively in team meetings, case discussions, and relevant training.
  • Promote equality, diversity, and inclusion, ensuring services remain accessible to all.
  • Adapt to service-user needs through flexible working patterns, including outreach.
