What you will do

• Manage a caseload of adults experiencing a range of mental health needs.
• Act as Care Coordinator for community service users in line with CPA and service requirements.
• Undertake comprehensive mental health, functional, and risk assessments.
• Develop, implement, and review person-centred care and support plans.
• Provide specialist Occupational Therapy interventions that promote recovery, independence, and social inclusion.
• Support carers and families through advice, education, and therapeutic interventions.
• Participate in a Duty Rota, responding to urgent referrals and emerging risks.
• Work collaboratively with multidisciplinary teams, partner agencies, and community services.
• Conduct holistic psychosocial assessments and identify strengths-based outcomes.
• Maintain accurate, timely, and comprehensive clinical records and reports.
• Support and mentor junior staff, students, and trainees.
• Engage in supervision, reflective practice, service development, and quality improvement activities.

Requirements

• Degree or Diploma in Occupational Therapy
• HCPC Registration
• Community Mental Health Experience: Previous experience working within Adult Mental Health Services.
• Care Coordination Skills: Experience managing caseloads and coordinating care plans.
• Assessment Expertise: Strong skills in mental health assessments, risk assessments, and care planning.
• Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
• Multidisciplinary Working: Ability to work effectively within integrated health and social care teams.
• Record Keeping: Experience maintaining accurate clinical documentation and reports.
