About The Role

We are a 34 bed home split into two areas. One area supports 11 individuals with learning disabilities and dementia, while the other supports 23 adults with mental health diagnoses following hospital discharges. We provide a step down from hospital, transitioning individuals back into the community. Some of the individuals we support enjoy activities like gardening and office work, and we love celebrating occasions with parties and fun events.

As a Support Worker, you will be at the forefront of providing high-quality, enabling care. Every day brings new challenges and rewards, making this a truly fulfilling role. You\’ll be key in helping us achieve our goal of being the best in the field.

Key Responsibilities:

• Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
• Develop flexible, engaging activity plans that promote autonomy and personal growth.
• Support individuals in maintaining healthy lifestyles and managing their health needs.
• Facilitate effective communication and maintain detailed, person-centered records.
• Oversee medication processes and support individuals in self-administration.
• Maintain a safe, clean-living environment and adhere to health and safety protocols.
