About The Role

This role is based within a residential care home, providing consistent support to residents in a safe and nurturing environment. The home operates on a round-the-clock basis, offering a range of shift patterns to ensure continuity of care at all times. This includes weekday, weekend, and overnight shifts, with staff working as part of a dedicated team to deliver high-quality care and support 24 hours a day, seven days a week

Key Responsibilities:

  • - Individual Outcome and Wellbeing: Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
  • - Activity Planning: Develop flexible, engaging activity plans that promote autonomy and personal growth.

  • - Communication: Facilitate effective communication and maintain detailed, person-centered records.
