This role in Stoma Care is to work within the Croydon Stoma Team to provide an expert Stoma Care Nursing Service to Croydon University Hospital and community as part of Croydon Health services NHS trust.

Main duties of the job

  • To assist with influencing, planning, organising, and implementing programmes of individual, core and strategic stoma care service delivery in accordance with trust priorities
  • To attend the hospital MDT as required
  • To explore opportunities to enhance patient care through nurse led initiatives
  • To attend and participate in regular meetings to discuss areas regarding service provision within the Stoma Care Team
  • To support the stoma care nurses in accepting referrals for and organising to meet/visit elective patients preoperatively to provide information and education to patients and their carers regarding their proposed surgery
  • To assist with siting patients for their stoma forming surgery or possible stoma forming surgery involving the patient or carer in the decision-making process
  • Provide patients/carers with high quality written information
  • To lead ward staff in developing a plan of care for each patient incorporating discharge planning
  • Provide post-operative care, information, teaching, counselling, and support to ensure patients and their carers have appropriate levels of knowledge and skills
  • Provide follow up care and support during home visits as required and in the Outpatients Department
  • To maintain accurate documentation and provide verbal and written reports as required
  • To support the Stoma Care Nurses in the management of complex cases, communicating relevant information and ensuring appropriate referral and management for these patients with the MDT

Person Specification

  • Registration on Part 1 of NMC Register
  • Membership of RCN
  • Full UK driving licence
  • General surgical/colorectal surgical experience
  • Experience of working in a stoma care environment desirable
  • Ability to work alone and as part of a team.
