Overview
The postholder will support the ongoing implementation and delivery of Personalised Care for people affected by cancer, specifically carrying out holistic needs assessment (HNA’s) with patients and producing associated care and support plans.
- To support the successful implementation of holistic needs assessments (HNA’s) for people affected by cancer across the NCA, assisting with the process of offering and delivering HNA’s to patients and producing associated, comprehensive care and support plans
- Provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs
- Providing health and well-being information and support in relation to their cancer, signposting to supportive services to enable navigation of health and social care; to empower the patient to manage their cancer.
- Work closely with the Macmillan Information & Support Service to ensure appropriate referrals to the service are made.
