Route 29 is inspired by the 'No Wrong Door' approach which is an exciting way of working collaboratively to meet the needs of our most vulnerable children and families in Salford. Route 29 will focus on providing a robust and asset based intervention to young people aged 8 -18 years who are on the edge of care, in care or at risk of placement instability. Route 29 embraces a close working relationship with adolescents by "working with" rather than a "doing to" approach. We want to be seen as a listening organisation and one where young people do not need to repeat their story to each professional, no matter which agency is involved.

Our service includes a 28 day residential home that supports young people at times of crisis. We have an exciting opportunity to join Route 29 as a Portfolio Lead for Future Journey's. This role has been developed to be a key component of the support we provide to vulnerable children and young people.

The role involves:

  • Overseeing all step down plans for Route 29, ensuring that there is identified ongoing support for a child and their family.
  • Working closely with wider services to ensure excellent knowledge of community resources and universal service offers.
  • Supporting the staff team to identify appropriate services to refer young people to when their time with Route 29 is coming to an end.
  • Ensuring that the progress they have made is sustained.
