Main duties of the job
About the role
We are seeking an experienced Registered Nurse to join our Advanced Care Planning Service, working in partnership with the NHS to support patients with palliative care needs. You will work autonomously to support patients and families to develop advance care plans, including advance statements, treatment escalation plans, ReSPECT documents and emergency healthcare plans. The role involves sensitive communication around future care, including preferred place of death, and close collaboration with GPs, specialist teams, hospitals, care homes and hospice services.
- Registered Nurse with current NMC PIN
- Degree in Nursing
- Significant palliative care experience (acute/community)
- Experience in advance care planning
- Excellent communication, organisational and record-keeping skills
- Ability to work independently and as part of a multi-disciplinary team
- Access to own transport
- Qualifications or training in palliative/end-of-life care
- Knowledge of advance care planning tools and SystmOne

Job responsibilities
Saint Michaels Hospice was delighted to launch a new person-centered advance care planning (ACP) service in July 2024 for people who have been identified as having a palliative diagnosis. The Advance Care Planning Nurse Specialist is a new role which through working as part of a Clinical Team will increase the quality and experience of care provided for patients with a palliative diagnosis and their families through the provision of integrated care and reducing inequalities.

Person Specification
- Registered nurse with current NMC PIN and a degree in nursing.
- Significant palliative care experiences in acute/community care.
- Evidence of experience in advance care planning.
- Knowledge of advance care planning.
- Qualification in mentorship/teaching certificate.
- Advanced communication skills/communication skills training.
- Diploma/degree in end of life care/palliative care.
